    On the application to register as a firearms dealer (La. Form)

    I am an individual so I mark individual. Do I then leave the following spaces blank?

    1. Trade or Business Name
    2. Name of Parish County where located
    3. Business location (physical address)
    4. And then what goes in applicant's business is:

    It seems like I would leave Business name blank for the mailing address my home address. The physical would be the same so do I need to put the physical in business address?
    And do I use individually owned or other for applicant's business since its N/A.

    Then on the Federal form Application to make and Register a Firearm:

    What do I put for:

    1. 4b Type of firearm
    2. 4d Model
    3. 4h additional description
    4. 4i State why you intend to make firearm

    I leave the FFL and SOT info blank since I don't have one.

    Its going to be a POF 416 lower and I have a 9.25" barreled upper that I intend to use when the tax stamp comes back. I guess the P-416 would be the model. I just want to make sure I get everything right the first time and get the CLEO signature when he does my suppressor so I don't have to go back.

    I put J15-F (I am doing a EA lower, and its says Model: J15-F) P-416 would be the model #

    4h is "your" marking.. J.Doe BR, LA
    4i "All legal purposes"

    When I talked to the girl at the state police , about an SBR, she said that you only need to fill out the form to register a firearm. This information is put in the section of " Firearm Data" in the block that says "Additional Discription or Data Appearing on Firearm" you just put that " I will personally be converting this rifle to an SBR".

    Three approved form 1s later all is good to go.

    I was told since the rifle was already manufatured, registered by another maker by way of serial number, that the other form is not required.

    If I buy a barrel shorter than 16" all I have to do is fill out the " Registar a Firearm" form(and all the other tax stamp stuff) because the barrel is already manufactured and not being "made" shorter?

    I filled out both forms and got both forms back approved. The way I understand it you are in fact making a brand new firearm when you make a SBR. But LSP might not see it that way.
